一、旨揭計畫為服務符合優先接受家庭教育服務之單親、已離婚登記或實際單獨照顧未成年子女者，及經評估家庭功能較薄弱且有心理支持需求，並育有未成年子女者，提供諮商服務及辦理家庭教育講座活動，若貴單位服務對象有此需求之個案，請協助轉知及轉介。
二、為避免資源重疊使用，若本（ 114 ）年度已有使用本中心「創造心生活，帶愛向前走」個別輔導計畫之個案，請勿重複申請。
三、本方案相關服務內容及轉介表如附件，請配合協助將旨揭訊息廣為周知。
