一、補發申辦流程如下：
(一)原已持有孔廟版面學生卡者：遺失補發申辦期限至學生 就讀之教育階段結束，未開卡(係指未開通一卡通儲值功 能)者，請務必提醒學生使用最新版之「臺南市數位學生 證票卡處理申請單」(附件 1)進行申辦
(二)未曾持有孔廟版面學生卡者： １、身分別： (１)國小 109 學年度入學之新生及市外轉入生(需符合 114 學年度畢業者)。２、辦理方式：(１)請有意願自費申辦之學生以郵政劃撥方式繳費(製 卡費 100 元+郵局手續費 15 元，共 115 元)並繳交個資 同意書。 (２)至學校填寫「個資同意書」並連同劃撥收據郵寄予一卡通(806 高雄市前鎮區中安路 1 號 3F 一卡通票證公司營運部)。
